Opt-In

Use this Opt-In Form if you believe you are a Class Member and you wish to remain in the Lawsuit and seek compensation for your individual injuries.

Please read the following statement carefully. If you wish to seek compensation for your individual injuries, you must check the box below AND complete the required questionnaire.

I understand that by opting in, I will remain a member of the class and participate in the Lawsuit. I understand that by opting in, I MUST complete the required questionnaire (which will be made available to me after this opt-in form is completed) by July 1, 2025 in order to seek compensation for my individual injuries.

Opt-Out

This is not a Claim Form. This is an Opt-Out Form that removes you from the Lawsuit. Use this Opt-Out Form only if you DO NOT wish to remain in the Lawsuit.

Please read the following statement carefully and if you wish to opt out of the Lawsuit, check the box below and provide your contact information.

I understand that by opting out of the Lawsuit, I will not be a member of the class or participate in the Lawsuit. I will not be eligible to receive any money or other relief that may result from any trial or settlement of the Lawsuit, if there is any. I understand that by opting out of the Lawsuit, I will not be bound by the outcome of the case. As a result, any rights I may or may not otherwise have will be unaffected by the case, and I will keep any rights to sue the Defendants separately over the legal claims in this lawsuit. I understand that, if I want to file my own lawsuit and have a lawyer represent me, I will need to hire my own lawyer. I understand that there is a statute of limitations that is as short as two days after I submit this opt-out notice.

Do Nothing

Do nothing. Stay in this lawsuit and await the outcome. If you are in the Class and do nothing, you will be bound by any final judgment, but you will not receive any money compensation from the lawsuit if you do not opt in, and you would not be able to bring a separate case if you do not opt out.
